As part of her bridging work, our bridge nurse is the link between the doctor or hospital and the care team of the children's hospice. She supports families during initial contact with the Sternenbrücke and is the contact person for the coordination of the takeover of terminally ill young people from clinics into the children's hospice.
Furthermore, she informs about the work of the Sternenbrücke in inpatient and outpatient facilities and self-help groups in order to draw the attention of concerned families to the relief care in the children's hospice.
